What this policy covers

This policy covers information collected through 10on10pt.com. It does not cover the protected health information created once you become a patient of 10 on 10 Physical Therapy. That information is governed by HIPAA and by the Notice of Privacy Practices you are given at your first visit.

What we collect

We collect only what you type into a form on this site. Depending on which form you use, that can include:

• Your name, phone number and email address
• What hurts, how long it has hurt, and how it is affecting you
• Your date of birth, ZIP code, insurance provider and member ID, if you ask us to check your benefits
• Whether you agreed to receive text messages from us
• The page you submitted the form from

We do not sell your information. We do not share it with advertisers. We do not use it to build a marketing profile of you.

Why we collect it

To call you back, to schedule you, and to answer the question you asked. If you submit insurance details, we use them to verify your coverage and tell you your expected cost. That is the whole purpose.

Text messages

If you check the consent box on the appointment form, we may text you about scheduling and your care. Message frequency varies. Message and data rates may apply. Reply STOP at any time to stop receiving texts, or HELP for help. Consenting to texts is never a condition of receiving treatment.

How forms reach us

Forms on this site are processed by our website host and delivered to the clinic by email, and to the practice management system we use to follow up with you. Those providers process the data on our behalf and are not permitted to use it for their own purposes.

Cookies and analytics

This site does not set advertising or tracking cookies of its own. Embedded third-party content, such as the Google Maps panel and the Google reviews widget, is loaded from Google and may set cookies under Google's own policies. Those panels load only when you scroll to them.

Keeping it safe

Information is transmitted over an encrypted connection and access is limited to the people at the practice who need it to do their job. No system is perfectly secure, so please do not send detailed medical history through a website form. Call us instead.

Your choices

You can ask us what you submitted, ask us to correct it, or ask us to delete it. Call (973) 453-0488 or email enquiry@10on10pt.com and we will handle it.

Children

This website is not directed at children under 13 and we do not knowingly collect information from them through it. We do treat pediatric patients; in that case a parent or guardian provides the information directly to the clinic.

Changes

If this policy changes we will update the date at the top of this page.
